The Medication Pathfinder Network , a provider of medication therapy management services, has enhanced the immunization tracking and reporting capabilities of its web-based proprietary software. Now pharmacists in clinical and ambulatory settings can monitor vaccines provided to patients from virtually anywhere there is a broadband Internet connection, and create reports that can be transmitted to the patient’s primary care provider. Medication Pathfinder also plans to introduce a wallet-size vaccine record for patients. The decision to expand the software’s capabilities was based on a number of factors, explains James Notaro, president of Clinical Support Services, the company that developed the MTM network and its suite of services. “The growing number of pharmacists who are developing immunize practices, coupled with the new Medicare Part D guidelines indicating that community pharmacists will soon be taking on a leading role in the in-network administration and distribution of vaccines not covered under the Part B formulary, spurred us on to enhance our immunization module,” Notaro says. Medication Pathfinder creates complete patient profiles by gathering patient medical condition and prescription data from several sources, allowing the pharmacist to provide medication regimens that improve care and reduce costs. In addition to the several modules of the Pathfinder program, pharmacists can provide their patients with port- able access to their medical records via web-based, personal medical records software through Clinical Support’s PMRSmart module. |
