Outcomes Pharmaceutical Health Care, located in Des Moines, Iowa, has been selected by Freedom Health to administer the medication therapy management (MTM) program component of its Medicare Advantage prescription drug plan.

Eligible members will receive MTM services from trained pharmacists to help produce safe and effective results from their medications, while controlling costs. Freedom Health is one of the many plans across the country to contract with Outcomes for MTM services.

One service is an annual medication review, where members of the plan meet one-on-one with a pharmacist in their area. This review looks for potential conflicts or duplication of therapy. Additional services, such as patient education, compliance monitoring, and assistance with nonprescription drugs, will also be available.
